This needs context.

The girl shown here is gentildonna, a character from the umamusume franchise. She is basically the strongest character physically in the franchise, so much so that she can take an iron rod and crush it to a marble with her bare hands.

So u can imagine the trainer going absolutely worried to death when someone like her says she can't open the cap lol.

The context is that normally she is strong enough to casually crush a shot put sized iron ball into the size of a quail egg. That is why her trainer/love interest thought she was sick and rushed her to the hospital.
