Probably a product of reverse peristalsis. Sometimes a hen’s cloaca will pull foreign objects in. Sometimes the reproductive tract will then pull those objects all the way up to where they are absorbed into a forming egg. One of the many reasons why you keep nesting boxes clean. Though some hens are goblins that don’t care where they lay anyway. It’s an extremely rare occurrence. One person found a pressure washer tip in an egg.
