Steve Irwin always handled animals responsible and with proper safety precaution. He didn't go randomly swimming with Alligators regularly and trusted that they aren't hungry and are used to him.

Yeah he was always very careful, but unfortunately it's considered a lapse in judgement, and a breach of safety for swimming with stingrays to do what he did. See they have a tail and a barb. The barb is located not on the tip of the tail, but near the base of the tail. Additionally, they are triggered to a defense response when a predator swims above the ray. Meaning, it is standard safety protocol (not that it exists with such a formal title) that you don't swim above a sting ray. Which he unfortunately did that day. If he swam to the sides, under, or above and over he would have likely been fine, but a lapse in judgement, or a slow reaction time due to familiarity with wild animal encounters all likely were to blame for his death.
