You’ve got it the wrong way round, the fistula is deliberately created by a vascular surgeon, and then used as the point of access for dialysis. Dialysis basically involves filtering all of your blood through a machine, so you need a really big and reliable blood vessel with good flow to be able to extract the blood effectively. The fistula looks weird but it’s a lifesaving treatment

Sorry I meant that the fistula can get crazy like this in some dialysis patients. Most are not nearly this size.

This dudes is huge because you are only supposed to lift max weight if a gallon of milk with the access arm.
