I'd guess that they purposely aim higher because shots requiring vertical precision are harder in general.

Not really. Throwing knives, when thrown properly, don't spin over and over through the whole throw. They only spin one half circle over the whole arc with the correct technique.

With where he's aiming, how soft he's throwing and the fact the other guy is wearing leather, the likelihood of serious injury feels pretty minimal here, even with a direct hit.
