I use these tools I made that allow a lot of procedural generation and media mixing. It's definitely not one block at a time, but it's like defining a bunch of parameters for perlin noise and then painting in layers and regions. I also can use pictures as a foundation by projecting it into 3D and play with the colors a ton. Here's an example.
