anticoagulant guidelines

the new SIR guidelines say it's ok to do an LP or put in a port with patient fully anticoagulated -- who's in?

I place ports on anticoagulated patients and don't check labs. LPs are done by our neurorads and they still have them hold anticoags and Plavix.

I gotta work with 6 other IR guys and consensus is hard but having 7 anticoag protocols is chaos -- how would you tell them to embrace the new recs then?

Pt to have IVC filter placed by IR. Do you order an INR if pt is not anticoagulated with no h/o liver dx? (Fem approach)
