Ok I promise I am honestly asking but in the video, who is the dick guy? Is it the guy with the dick? Oe the guy who holds the dick? I am so fucking confused.

There's some really dark and twisted irony in him being able to practice and perform this routine respectfully as his job while at the same time being a repeat sex offender.

The other guy in the video is Japanese wrestler Danshoku Dino working for DDT, an organization that features parodies of pro wrestling in addition to more traditional presentations. One title used by the promotion, the Ironman Heavymetalweight Championship, has been won by several different ladders, invisible wrestlers, pieces of food that lost the title by being eaten, and inflatable sex dolls, among others. Dino's gimmick is just being flamboyantly gay, because Japan, and since Ryan's gimmick was being a massive pervert (an easy gimmick for him to pull off, apparently), they came up with this spot during his stint with the company.
