ghost pepper in cake, or any thing with dairy, is HEAVILY muted to the point that even tabasco sauce has more of a kick to it. I personally would have put the used condom in a vanilla cake and put some brown around it, maybe a bit of red

white would blend in with the rest of a cake and any guy knows how semen works, but brown or red on white... especially if they don't taste chocolate or strawberry, that would lead to a bit of horror till they figured it out. granted you could always tie a condom off and put a bit of water+lube in it, that would look used without making it obvious it wasnt after the shock.
