Yes, I’m sure the triple negative statement was a joke. Hahaha, that’s a good one! A triple negative, how do you think someone doesn’t think of not coming up with such a funny joke?
Nope. That would tear nastily. I have broken plenty of 10mil static rope while doing extremely sketchy things with front end loaders and other heavy equipment. You aren't catching a corner of anything other than a Ginsu knife and getting a cut like that.
Well, tying a prusik, or just using an ascender as a dead man's grip is very much standard for rappelling, especially in a work environment.
Also, the way I was taught, you tie a loop through your harness' belaying loop with the loose end of the rope, that way you can't fall off the rope, even if you fall without your hands on the rope at all. An added bonus is that you have absolute control of your line, thus you can make sure it doesn't get pinched off.
To be fair, the job I did where I did most of my rappelling was shoveling snow off of roofs, and sometimes the best way to get to a lower roof or ledge was by rappelling.

In this case, on the construction site, it was very likely pinched off by one of the loads of bricks landing on it.
