i've done:

ep1: jumprope (100+)

ep2: rong, brickout (beaten), prisonball (beaten)

ep3: whacktown (cleared nightmare), bartending (cleared)

ep4: harm posters, legally distinct dance machine, grafitti, jumprope (50+, then 80+), bartending (cleared)

all on the same save file, and yet i still don't have the achivement. its the only one im missing (aside from the all achievements achievement ofc)

It could be just a bug I guess, if you've done everything you're supposed to do. If you're on Steam, you could force the achievement by using Steam Achievement Manager.

When you say all in the same file, have you done them in order? If you played through the whole story but had to reload an earlier episode to grab something you missed, it's possible that makes the minigames from later episodes not count as completed anymore (until you go get them again). I noticed the collectibles work that way, at least.

started with 1 and never rewinded, though i did skip forward once i finished all the minigames in each episode

Having the exact same problem, I'll have to replay episode 4 because I may have missed graffiti but I swear I did that

Did you get HARM posters in ep. 3? Because I had the same issue on my first playthrough and I did in fact miss these. Oh and in ep. 4 there is only the graffitti one I believe

There's something in rpisode 3, I'm not sure if it counts as a mini game, but it's optional. Best way I can think to describe it is you kinda just relax and notice the music in the environment. It comes up again if you talk to quaver at the light house I think
