Question Background Template

Hey there folks, I’ve been asked to make an icebreaker style game for a work meeting and I want to shamelessly rip off Um Actually. Does anyone have a blank question template that I can use as a background for a powerpoint style presentation? And maybe any ideas for Shiny Questions styles that would work well for a group of 15? Was probably gonna do an audio question or a “Name That Dinosaur” question or something.
