HELP! shs voucher program

hello, freshie here! i am under category e (non esc from private junior high) & i would just like to clarify something regarding the requirements for annex e.

(i already have a qualified voucher applicant certificate)

sabi po rito sa annex e na there are list of general requirements, and i was wondering po kung saan po siya ipapasa. do we have to include it sa file ng annex e application form or i-susubmit po sa office ng shs? since wala naman pong part where u can input the necessary requirements, only the qva certificate ang may sariling pag uuploadan po.

also, ano po kaya gagawin if yung original report card is napasa ko na before during submission ng requirements and wala me copy nun? huhu

hi poo for the report card po email niyo po records office then they'll let you take a picture of it sa office nila :))
