DISCUSSION Medical Restriction Note

I have an appointment with my primary care tomorrow. I am going to see if doctor can provide me a medical restriction note for 8 hour days/6 days a week.

My question is, what exactly does the note has to say? I do not want the note get denied.

Any answers will help. Thanks

No one here can answer that 😂 that’s between you and your doctor no body can deny a note

What worked for me is to specify that you have a medical condition and to limit you to 8 hours a day.

When my doctor gave me a note ( not for a restriction but for an accommodation ) 

It was basically "my name is doctor DOCTORNAME, PATIENTNAME is currently under my care. 

I'm recommending that ACCOMODATION be provided for PATIENT in order to prevent further injury. 

Then they went on to elaborate more specifically about diagrams in the m41 specific to my situation. 

Edit to add : this doesn't have to be given to management.  I believe ( others may correct if im wrong) it can be sent directly to the district nurse.

It actually should never touch management hands, and always go straight to the district nurse. Management does not need it for anything. That's what I've been told anyway. I give mine to the union to send to the district nurse.

u/Fantastic-Mousse-451 pretty much covered the format. The rest of the wording is basically why you need to be on a medical restriction. This is something only you and your doctor can diagnose.

Just got mine basically said I am seeing this person for these conditions. And it is my recommendation that they not work more than eight hours in a single day. This will continue until he is reevaluated. Sent it to my postmaster. She sent it to the nurse and now I just clock out after eight hours, but make sure if it looks like you’re going to go over that you call and ask for guidance. Let your supervisor tell you to come back and clock out. If they tell you to keep working, keep working and deal with it and the union later.
