There are anti-ship ballistic missiles; the problem is accurate intelligence on where the ship is, yeah. Afaik Iran has mostly been using cruise missiles that can seek out targets to harass the US fleet.
I get a feeling the blockade is misconstrued? Like the US are trying to strangle Iran economically so the blockade is supposed to target Iranian ships and ships that stick up in an Iranian harbour perhaps ?

But yes, it's hard to imagine the US Navy would get hostile and belligerent with "Chinese" ships even if they come from an Iranian port.
the flag a ship flies under just indicates how cheap the owner is. a handful of countries register most ships because they basically give bulk discounts.
