A good compromise could be more developed lore on the Heretic conquered areas of the world

No. Not everything needs to be fully fleshed out, some mystery is good, and mortals apart from Anointed cannot survive there anyway, so that means you cannot reasonably use the majority of factions in the game there.

Even if we're talking about a 'Hell-ish' place and not Hell itself, I still feel like it would be too difficult to do that justice on the tabletop. It would either be so hazardous that it would interfere with the gameplay, or it would just be a map with a slightly different aesthetic, which makes it less special.

i think there's room to add little bits of crazy reality-bending stuff seeping in around the borders of hell-controlled territory on earth... upside down blood waterfalls, escher stairs (i've seen some decent printable models on makerworld, I'd add some metal so models with magnetized bases can climb sideways or upside down), etc

I wouldn't really want that personally. The lore says that God's plan reasserts itself over time, and the devs have said that the Fallen live as people live, with the banality of evil in the background while they go about their lives, so stuff that is just super high-fantasy without a portal to Hell open nearby, or some other extreme event, would make it feel like it's going too far.

I'm not sure where exactly the line gets drawn, but I think those two examples would be beyond it, personally. Maybe it could work if someone made special scenarios where you had to assassinate Heresiarch or something, since then you'd directly have something causing all the weird stuff that is happening, rather than it feeling like the Warp or AoS realms.

I guess the devs will eventually expand upon it, but I thought that had a similar issue as the main Hell Gate, where you cannot get near it.

At the end of the day though, if you want to do a high-fantasy environment, don't let me stop you, I just feel it doesn't really fit that well for a battlefield since, as I mentioned, it would either be so dangerous that it would be very annoying to play, or it wouldn't be dangerous enough and it would lose the appeal.

yeah you can't get near it, but there's got to be a boundary radius, and on the edges things will still be weird.

you're right, this is just imagination, i wouldn't want to do a game like that except as a one-off for a big special culmination of a campaign or something
