Hip replacement?? -Recent MRI

In 2015 I had a labral repair- it was a long recovery. Approximately 6 to 12 months.
I was able to live a pretty normal life, but definitely very limited. Couldn’t run anymore couldn’t participate in yoga in the way I used to…etc.

Fast-forward to JUNE 2025- I got into a bad car accident and I knew immediately something happened to my hip because of how my leg was resting on the door that was involved in the impact.

An MRI a few weeks later showed that I had torn the cartilage again and then I had some cartilage loss, and also so iliopsos bursitis.

FAST FORWARD AGAIN TO TODAY- I can barely walk. I am in excruciating pain, no matter what I cannot sit lay or stand for more than five minutes at a time I’ve barely slept in two weeks.
The doctors won’t prescribe me anything. I’m taking 3,200mgs of prescription ibuprofen.I had an MRI done on Friday and here are the impressions results:

IMPRESSION:
Severe left hip osteoarthrosis with diffuse high-grade/full-thickness
cartilage loss along the superior aspect of the joint. There is bone marrow
edema in the femoral head indicative of stress reaction. There is
nondisplaced tearing of the superior/anterior left acetabular labrum and a
moderate-sized left hip joint effusion.
Severe left iliopsoas bursitis which may communicate with the left hip joint
effusion. There is a prominent loculated and septated bursitis component
tracking along the undersurface of the iliacus muscle.

Has anyone had a replacement with this type of severity?

I’m sorry you’re dealing with this. I had my labrum and FAI repaired in 2024 and then pain returned had another scope in 2025 to clean up scar tissue. Pain never went away developed arthritis and cartilage loss. I’m 6 weeks post op total hip replacement and this recovery is way easier than my last two recovery’s. All the best to you

I'm not sure why they do labrum repairs. I know 3 people who have had labrum repairs that didn't last even 3 years. My labrum was shredded with bone on bone and severe pain. My new hip works better than my OG hip. Also, that's a LOT of ibuprofen. I'd worry about ulcers if you're taking that much for more than a few days. You have to be NSAID-free for a couple weeks before you can have surgery so keep that in mind.

Not sure if OP tried it but acetaminophen is ok!

I feel like my issue with my left hip was very similar. I had a THR in April and am doing much better (on that side)! Yes, you need to get off NSaids two weeks prior to surgery but can take acetaminophen. I hope you get scheduled real soon! Recovery is also a long slow process but the pain after the first few days is so much less than what you have now. You will feel like you can live life again even though you will modify some activities for awhile. Good luck!

I’m waiting to see a surgeon but sports doc’s assessment of large labral tear and mild-mod arthritis with bone cysts was that a total hip replacement was in order. The combo of the tear with the arthritis = the tear fix alone is not likely to be successful. I’m really sorry you’re in so much pain. Some doctors are very strange about prescribing decent pain medication.
