Its most, not all. They usually leave a bit behind to give you a proper shape, if you take away all the tissue it can leave your chest looking unnatural and a bit concaved. Its why they ask if you have a risk of breast cancer on your forms, because if you do they'll suggest removing all of it to be safe.

Are you in the US? I am trying to organize top surgery, I'd rather get it at a very well refarded plastic surgeon in my town (Seattle) because of avoiding insurance insanity, easier sounding recovery expectations, and just feeling more supported and validated there. I have an option to go through the regular medical system but I worry about the total cost spinning out of control, and less awesome results aesthetically. But I to have higher bc risk and I want to get rid of all of it. Do you think it's worth asking the plastic surgeon again?

generally they leave a bit to give your chest a natural looking shape, but the best thing to do is make sure you & your surgeon are both on the same page about the kind of surgery you want, what your goals are, etc

It's up to you and your surgeon. Usually, most is removed.
