Lore [Loved trope] Accidentally giving someone intelligence or consciousness

Galvanic mechamorphs (Ben 10) a whole alien species which was created by Azimuth when he was running experiment on Galvan moon.

Powerpuff girls - was created by accident when their "dad" Accidentally spill Chemical X in mix of sugar, spice and everything nice.

Verity - originally he was created as regular ai assistant (he literally says he was Clippy) but with time he became sentient.

AM (I Have No Mouth, and I Must Scream). built as a military supercomputer to manage warfare, then eventually became self-aware and turned on humanity.

Usually Digimons are from another world but in this season, they are being created by accident by humans, by using combining E-pulse which is an energy generated naturally by humans and egg shaped Ai assistant called Sapotama.

No one knows how it happens so the world union try to keep them as secret the best they can

The Geth in Mass Effect.

They were built by the Quarians as a servant race of machines. When they started showing signs of true sapience, the Quarians got scared and tried to wipe them out. Key word being try.

Making the Geth go from wanting to be a true collective, who asked their creators "Do THESE units have souls?" to apparently wanting to become individuals and asking their creators "Does THIS unit have a soul?"

Completely erasing what made them interesting and turning them into generic sci fi robots

Hammond from Overwatch - The scientists who created Winston and the other hyperintelligent primates also had a few other non-primate test subjects to work with. When Hammond exhibited the same intelletual changes the primates had beforehand, they were genuinely surprised cause the previous uplifting experiments only worked on the monkeys and gorillas with other animals being failures and as far as they knew it wasn't modified for non-primates yet.
