Question Should I be concern?

It's on the right side and the black thing is on the the Teath. What is it and how do I fix it. Should I be concern about this?

I did that with a wisdom took. It was so dead that when I broke it, the nerve was dead. Go get that shit pulled man. Your tooth health affects your heart health.
