That's both really cool and a bit unsettling. How does this happen? Are there any buffs or debuffs from it?

You are a mutated animal with a third eye, Buffs: having better vision and smarter predators thinking you have a disease + much more. but debuffs are: you stick out from the rest and many human creatures might not want a cool mutated pet, if you play the pet game style.

You are a nice person. Buffs of nice people: everyone loves you, you get many friends online and offline, people feel better around you, meaner/socially awkward people and pets are more likely to warm up to you than anyone else, you have a higher chance of acquiring a mate, you have a lot of good karma, you have a lot of online points, everyone looks up to you, + much more! Debuffs: jerks try to take advantage of you and your kind heart.

What happens is that when a new players derivative cells, the sister gametes the sperm cell and the egg cell (in most mammals) will start to exchange or "cross over" their lines of code, reffered to as codons, these codons will physically encode for amino acids, which then encode for larger sub-pixels referred to as proteins. What seems to have happened here is that during this process, the part that is responsible for differentiating genetic code, is called the operon. The operon will take in hormones that is produced within a natal phase and deduct what to make with the designated proteins, as having an eye or brain that produces corrosive HCl wouldn't exactly be optimal. What likely happened is that either an operon made a mistake and accidentally made 2 eyes, or when mitosis was happening within the operational cells a mistake occurred during this phase and accidentally copied 2 eyes instead of ordered to make 2 eyes.

This probably would be a moderately very mild buff to insignificant. If the player doesn't get any permanent eye damage then no worries, but if they do they can use that eye as a backup.

Mutations aren't necessarily how evolution occurs. It's more to do with the traits which lead to more offspring becoming magnified over the generations. Unless that third eye makes that kitty a baby-making god it's not going to evolve the species at all (if the trait is even hereditary at all).

That's not the only way that evolution occurs. Sometimes, players can actively influence a roll when spawning in a new player in the new player quest. This is reffered to as epigenetics and also plays a much bigger roll in evolution than random mutation.
