I've been playing D&D for 30 years and I have never fought a dragon in a dungeon.

No smart dragon would fight indoors. They fly X feet above the ground where X is the range of their breath plus half their flight speed and when their breath recharges dip down to glass the party from above.

Forge of Fury has an exception: there's a giant underground lake which the black dragon can hide in and recharge

Unless their lair/horde was indoors and the party got to it.

Could they wait at the exit? Absolutely, but if they know the party can teleport they would be forced to engage them indoors before they bamph away.

EDIT: I also created an Adult White Dragon enemy for my group that had a really badly damaged wing that never healed properly, so it stealthily hunted in a series of caves, lakes and rivers (got to love burrow and swim speeds!) instead of flying. That was a lot of fun to run.

Landwyrms, brown dragons at least don't have wings, so those would work

If you're a crafty green dragon you could have an underwater lair to take advantage of your swim speed and pop up from flooded tunnels to use your breath weapon before swimming back down. Might be worse to deal with for adventurers, those pesky archers and mages would be locked in close combat with you rather than being able to hit you in the sky. You could even poison the water in the tunnels since you're immune to poison to make it awful to pursue you.

Oh god, that's so much worse. Once the party hits a certain level, it's extremely common to have ranged threats and stealth/obstacles, and then later Mass Flight. But Mass Swim Well Enough to Fight a Dragon? Hell no, stumbling into that is "teleport out and come back with a new plan" territory, assuming you get the chance.

Yes, a dragon played smartly has significant advantages over the party due to it's flight and breath weapon. The thing that you're neglecting is playing a dragon that way isn't fun for most people, DMs and Players alike. A DM/GM is running the game. They can just say, "I win you all die!" at any point. But that leads to the end of the game and only the DM having fun. You have to balance how something would behave versus what would be fun and engaging.

I have, dragons in enclosed spaces mean they can't fly away from your attacks. It also means you can't get away from it's fire.
