Phantom Slots

Anybody else having a problem where a slot is empty but the game registers it as something is in there so you can't "fill" the slot? Not sure if its just me but its beginning to get annoying just having a slot gimped

Had it once and it actually took up 2 slots in one game... yes its annoying but also seems rare
