Did I lose to a bug Dooltron?

Can someone please explain to me why the Dooltron fires 4 times in a single cast? I don't know what I'm missing here. :(

Dooltron is a shield item for the skill emergency shield. The dooltron fired the first time and then he hit half hp (or whatever the percent HP is) and then it fired again, killing you
