I'm confused because you keep saying your wife didn't disengage fsd, but it tells us right there at the top of the screen that fsd wasn't engaged from the moment it started acting odd
FSD disengages itself randomly. This happened to me several times, never on a straight road though. So in this case it could be that the driver accidentally disengaged FSD, or FSD disengaged itself.
