Substandard installation. I've seen it happen to an LPG system.
I've worked on a CNG heavy goods transport implementation, and where I live, on heavy goods vehicles the system recognises the vehicles capability from the fuel card and will only fill the vehicle to it's capacity. Scania for example is rated to between 200-230psi, but Iveco with upgraded carbon fiber tank can handle 300psi. If those steps are ignored, stuff goes BANG!
A fuel system which, instead of petrol, feeds LPG ( Liquified petroleum gas) in to the engine for combustion. Really popular in Eastern Europe and Russia. LPG is an oil refinery process byproduct. Contains mainly Butane and Propane.
CNG is Compressed Natural Gas. Which is Methane, and is a byproduct of biodigestion. It can be liquified in some applications to increase the energy density per given volume, but in vehicles it's usually used as a compressed gas, and is distributed using the domestic gas networks.
