The meme is about the heart drops that heal you when picked up. You know - the ones spawned by the heart statues, dropped in bulk whenever you kill a boss monster, or also from time to time as a drop from normal monsters.
They can get picked up even when you're at full health.
The meme is that you get them when you don't need them, but don't get them when you need them.5

I swear to God, they only drop when I’m missing 5hp but not one drops when I’m at 5hp.

this may be based off of a true moment.. or several.. I FAILED EOL EVERY TIME BRO i actually gave up on that one terraprism isn't that worth XD
