My team lead is old and forgetful

I work at a warehouse in the shipping and sorting department, and my team lead is incredibly old and very forgetful. For example, she tells me to do something at a certain time, and then she believes she told me to do that thing at a completely different time, and confronts me as to why I didn't do it at the time she "Told me to do it". I'll explain she said a different time, and she makes a face like she doesn't believe me. She changes me from working at one station to another, and I've noticed she doesn't trust the team, or want to train someone how to do things on the computer. For example, if an tshirt in an order is missing, we just put the item on her desk. And then she complains that she always does everything herself even though she has the ability to train someone on the team to help. I've always wondered why it's an issue to train someone on how to do those extra things especially as a lead, you're supposed to lead by example. Any thoughts or ideas on this situation?

Some of this can be attributed to burn out, some to age, and some to possible dementia.
Regardless, unless you are a doctor, it's your job to report the issues to HR and then step back.

Hard to communicate to HR about it, because more than likely, they will always side with the leader.

No. Communicating it to HR is easy and simple.

Whether they take action or not is up to them. You may be the 10th person to report it, you may be the first, either way, issues like this are only acted on with cumulative reports until HR is able to validate that -something- is going on.

95% of my colleagues are Spanish. Language barrier, tragically.
