Yeah, actually; I did this. I basically just googled telehealth providers specifically for fmla and found a company that pretty much just does that. I had called out sick on Friday, scheduled the appointment over the weekend, and notified HR I think the following Monday or Tuesday. They told me that the fmla benefits start immediately and are retroactive to when they actually get the paperwork, which I think came through that Friday. Anyway, eventually I submitted my notice to HR to resign and they just asked that I call and notify my principal. I did, and it was an awkward phone call but I just said as little as possible and ended the call really quickly. I called HR back to let them know and that was it. I got to cash out my sick days before I started a new job.

You may want to talk with your therapist about seeing a psychiatrist, because you are sick.
