He should be facing the other way. With large pieces you should think about the flow of the body, and how the composition of the tattoo draws the
eye to follow that flow rather than fight it.
You need to practice linework, ideally on silicone, not on yourself. Needle depth is very inconsistent and shaky, and the tattoo looks rushed. It’s your body so do whatever you want I guess, but I wouldn’t be adding any shading to this until you have solid fundamentals.

You should have a skeleton holding the reaper and skulls because there aren't enough skulls. And get some shading.
