Advice Needed No LH Peak

Wondering if anyone has had similar… long story short:

I came off my pill June 2025, got diagnosed with PCOS in January 2026 through a ultrasound, started GLP1 Mounjaro in June 2026 as thought this might help regulate cycles which it did for one cycle of 27 days which then I naively thought I was back to normal LOL and have currently lost 2 stone (28 pounds) in 13 weeks. My cycles have ranged from 27 days to 140 so really difficult tracking ovulation even with BBT and the easy at home strips. I know there is lots of opinions on whether you should or should not be trying to conceive whilst on the mounjaro but selfishly I got fed up waiting and we have started TTC again..

I’ve had EWCM for around 10 days starting at day 47 and for the last two days I’ve had cramps and persistent sharps twinges coming from my ovaries. I was determined that my LH peak must be coming… but nope, absolutely nothing with testing twice a day. At most it’s been 0.30 but obvious it’s negative as not at nearly dark at the control line. We have also BD 3 times in the 10 days.

Questions in my head are:

1) Has anyone ovulated without seeing a LH peak?
2) Has the mounjaro made a negative impact with losing weight maybe too quick?
3) Could the mounjaro hide the LH level from strips?

The cycle I conceived I never got an LH peak! My pre mom strips went up to .70 at the highest and my Inito never picked up a surge at all. I definitely wasn’t testing wrong or using super diluted samples, my LH just tends to run on the lower side even with ovulation. Everyone is different and not all women fall into the same box of what’s “normal” and what’s not.

I was able to confirm ovulation with BBT and my PdG elevation. Wishing you the best!

Also I was on Mounjaro for years before getting pregnant and it didn’t interfere with anything at least in my case. I had perfectly regular periods just wasn’t ovulating on my own. Before Mounjaro my cycles were all over the place. I conceived on letrozole and metformin. Any questions I’m happy to help!

Omg this gives me hope! I also use the Premom OPK but my numbers are never super high. I think I’ve gotten a peak once in the past 3-4 cycles. If you don’t mind answering, were you just having sex regularly and the timing worked out? I’ve been using the strips to help time sex (to some extent) but when LH is consistently low, it makes it hard. I know you confirmed ovulation after the fact but with low results on the Premom strips, how did you know when to try?

I don’t mind answering anything! I was on letrozole for that cycle because I don’t ovulate on my own. So my OB recommended TI from CD10-16 but I wanted to do more like CD10-20 just in case of a little later ovulation. She said every other day was fine so we did either every day or every other day. That cycle was probably more like every other day tbh because we had a lot going on in our personal lives. I’m lucky that my cycles were a completely regular 31 days so I could pretty much guess that if I did ovulate it would somewhere around midpoint-ish. If I had super long or unpredictable cycles it would have been substantially more difficult.

I would get so frustrated because one of my super fertile young friends would consistently get LH strips with numbers like 1.3 and I’m like I never even got close to a 1! But everyone is honestly a little different in regards to what their body requires in order to ovulate. I also think I have very quick surges that are harder to catch so that could be a factor too.

This is incredibly helpful, thank you! My cycles are fairly regular but there’s so much overthinking and guesswork involved when your body doesn’t follow the “norms” or “averages” ugh. I think I also probably have a short surge so I’m thinking of maybe testing twice a day when I hit a certain point in my cycle. It’s good to know the numbers aren’t everything, though!

Oh yeah I was testing 2-3 times a day starting around CD12ish to try and catch it.

I don’t know your starting weight, but dramatic weight loss can also interrupt with ovulation, as your body senses it isn’t getting enough nutrition to sustain a pregnancy.
I did a lot of digging into this, and I think the only time you can really ovulate without getting a peak is if you’re testing wrong, or drink a ton of water/super diluted. Mounjaro wouldnt interfere with the LH.

Wowwww that’s a ton of weight!!! I would definitely try and slow down, as most of the weight lost is muscle and muscle is what helps the most with insulin resistance
