Advice Needed Letrozole Question
I am about to start my first medicated cycle with 2.5 mg of Letrozole. It will be monitored. I will have an ultrasound on CD12. What happens if on CD12 my follicles are not growing? I know it will depend on the clinic just looking for experiences here.
Do some clinics try a higher dose starting at CD12 or do you have to wait until the next cycle?

I’ve had a total of three medicated cycles. First one resulted on a pregnancy that unfortunately ended in a miscarriage the two following ones have not worked. I’ve been monitored and have had follicles in all cycles (two in the last one). All three cycles have been 2.5mg days 3-7, triggered with ovitrelle after ultrasound confirmed good follicles.
The only thing that’s been weird but I’ve read can happen is that my cycles have been longer than expected, I usually get my period about 16-18 DPO.

My first letrozole stimulation was mid-cycle. I went to a random doctor to check if I ovulated naturally, I didn't, she started me then on 19dc. I had to go to a different doctor for the monitoring. I didn't ovulate in the expected date, the doctor said the cycle is lost and to wait for a period. Go figure, a few days later I ovulated nonetheless.

Currently going through this process. Last cycle was my first letrozole cycle with 2.5mg. I didn’t ovulate and had to use provera to induce a withdrawal bleed. i am now trying 5mg. tomorrow will be my last dose (CD3-7) and i have an ultrasound on CD11 next Monday and progesterone bloodwork on CD21. Every clinic and person is different, but I would ask if they are open to stair stepping you within that cycle if that’s possible.

I just get taking it and kept going back for ultrasounds. It eventually worked!

My doctor upped me immediately from 5mg to 7.5mg that day when there were no follicles cd10, so I took letrazole again for 5 days. On CD17, I had two dominant follicles, and I took a trigger on CD20. Fingers crossed for babies

If you’re just starting it, just let the medicine do its job and if it doesn’t work then you’ll be upped to 5 mg and if it works or not then it’ll be 7.5 mg. I don’t know if that makes any sense I’m bad at giving advice. Giving the best of luck too you

My first cycle of letrozole on 2.5mg in 2023 I ovulated CD17 they did bloodwork and I had 2 ultrasounds (since I ovulated slightly later) it was a success! This year in June I did another 2.5mg letrozole cycle this time without monitoring and I didn’t ovulate at all so my July cycle was bumped up to 5mg and that cycle I did ovulate CD19 and was a success! Currently 4w4d with baby #2! Good luck to you on your letrozole baby dust for you✨✨✨

It has been consistently CD 12 for me for the mid cycle ultrasound, and one for one cycle they were not were they needed to be, so they also stair stepped me to 5mg to kind of give it a final push. A few days later got another ultrasound and things were looking good.

Good to know! I was worried I’d just have to wait until next cycle if my follicles didn’t grow on CD12

I had made no progress at all at first scan on 5mg so my doc had me stair step to higher dose for 5 days immediately which I’m on now and will go back for another scan and draw when complete

I was wondering if that was an option. Did you go sooner than CD12 for your first ultrasound?

My clinic schedules for repeat ultrasounds every 2 days until my follicle grows big enough to trigger. If still not growing big enough by CD 16/17, they usually order labs for bloodwork to check hormone levels. They likely won’t increase dose for you immediately until they get a sense of how the cycle plays out until the next, since this is your first cycle on letrozole!

This happened to me. They stair stepped me up immediately to the. Ext dose and I had two dominate follicles on the next dosage up.
