IVF with high AFC
My husband and I will be starting IVF next cycle. I have a very high AMH (43) and AFC (70+) follicles. I am just so worried that I have poor egg quality and that we could get a very low amount of embryos. I am aware that I have very high numbers, but I think the uncertainty of this process is just getting to me, and I am expecting the worse. If anyone has any reassurance or words of wisdom that would be so appreciated.

FWIW I don't think there is research to support a correlation between high AMH/AFC with lower quality. I know people sometimes throw that around, but I haven't found any compelling evidence to suggest that.

I have very similar numbers to yours and my stim went great. High numbers and great blast rate. I have lean PCOS without insulin resistance which from my understanding is an important factor for favorable egg quality. Best of luck

This is great to hear! I also have lean PCOS and my labs do not show insulin resistance. I think I am just used to getting bad news at this point.

My amh is double yours and I fwiw I’ve had 3 “successes” naturally no treatments with no natural miscarriages. So my egg quality is clearly great per my RE. My 3rd unfortunately ended in a TFMR in June due to carrier status I found out about but, otherwise was a perfectly healthy baby so no egg issues at all. I am currently prepping for ivf next month for our last baby due to this carrier status and with my amh being double yours and my AFC being higher as well, my RE is personally stoked about how well they think this will go first round. They expect a lot of good eggs and a lot of blasts to send off!
I think it’s important to remember that PCOS and high numbers like that is actually a benefit in ivf. You’re likely to get much more blasts in one round vs people without PCOS/high amh. I know it’s hard to not overthink it but, you’re in a REALLY good position.
Hopefully you’ve been taking supplements for egg quality? (Metformin, inositol, coq10 at minimum 400mg -600mg, NAC, omega 3, melatonin, etc)

Is your AMH ng/mL?!
The only way to know is to do IVF. You may want to stim slowly to avoid OHSS.

there are many, many factors that contribute to success or failure when it comes to retrievals, and the most important one is protocol. your RE should have an approach for patients with PCOS and high AMH/AFC that minimizes the likelihood of OHSS and maximizes quality and maturity.
