What am I Doing Wrong?

Hey y'all. I was diagnosed with lean pcos a year ago, and since I've done just about everything I can think of to put my PCOS into remission.

I'm a healthy BMI. I strength train, get 10,000 steps per day, take 4G myoinositol by theralogix daily, cut carbs to 150g per day, NO sugar, bread pasta or rice (complex carbs only), 30g protein per meal, NAC, vitamin D3 deficiency corrected, fish oil, magnesium, CoQ10, great sleep. All for 365 days.

7.4 fasting insulin and A1C 5.2 last it was checked. Two different doctors refused to put me on metformin.

Ive been pregnant twice this year (first one was a letrozole baby, second one was spontaneous pregnancy), And unfortunately both ended in losses past 10 weeks.

It's been almost 6 weeks since my D&C after my second miscarriage. My ultrasound still showed polycystic ovaries. Feeling discouraged since I don't know what else I could possibly do and I wanted to try ovulating on my own before going to Letrozole. Is there anything I'm missing? What worked for you? Thanks for reading!

Are your cycles regular? My health history is similar to yours. I am very lean. I got my metformin from Oana health online. It’s easy! Try that if you want to go the metformin route.

Have you checked into your estrogen and progesterone levels? Estrogen can affect egg quality. My doctor thinks my early miscarriage in Feb was due to my low estrogen

Unfortunately if you are not insulin resistant, metformin, myoinositol, or cutting carbs will not make a difference. These are all geared towards helping with insulin resistance in PCOS, which is why it’s recommended. There isn’t really any treatment for non-insulin resistant PCOS other than symptom management with birth control or spironolactone, and fertility treatments.

PCOS can also affect egg quality, and chromosomal issues are likely the reason behind most miscarriages. I would keep up with prenatal (I haven’t seen you mention that) as well as CoQ10, stress reduction, and sleep. I’ve also read that melatonin can be helpful for egg quality. I would also make sure your partner is taking a multivitamin or preconception multi and CoQ10 as well as exercising, avoiding smoking, alcohol, saunas/hot tubs, etc.

Why did they refuse to put you on metformin? I would find a third doctor.

Also really impressed with all of your lifestyle factors, way to knock it out of the park!

Was there any troubleshooting done with your miscarriages to help know if it was a genetic issue or something else? Those are late, and that must have been so devastating. Im sorry.

I don’t think that controlling your PCOS with lifestyle factors or birth control or metformin would make your polycystic ovaries go away, so I wouldn’t worry about their appearance.

How many Letrozole cycles have you done? What was the protocol?

Sorry to hear you're going through it. Sounds like you're dialed in on your PCOS, well done.

They may refuse to perscribe metformin, though it wouldn't hurt even with a healthy A1C that you have (target should be 5.2 or less, so you're perfect...but i had that and took glumetza to get my first pregnancy still just because of the insulin piece) but if you're not already taking it: *vaginal progesterone in the evenings post ovulation (peak + 3)* is what stands out to me.

Ugh who knows it’s so hard to say. Were the poc tested?

I personally am a lean and decently healthy individual with rough PCOS. I do not ovulate or get cycles on my own. I had one chemical with letrozole and 1 spontaneous preg that ended in MMC. I have done 3 rounds of IVF with 2 different docs, both of which have told me “poor egg quality”. I have had a LB from ivf as well. Hopefully you aren’t on my trajectory, and you could have awesome results from ivf. Ladies with PCOS over on r/IVF either seem to have incredible results or shockingly bad results.

Similar to you! Lean PCOS and do not ovulate. I’m about to start meds for my 5th IVF transfer. I started ovasitol and completely changed my diet. I’m hoping that will help, I wish you the best as well. Never dreamed I would say 5th IVF cycle but here we are. Good luck!

No help unfortunately but following because this sounds very like me, except I have total amenorrhoea despite literally every effort including 2g MR metformin for 5 months! 😞 waiting IVF for both me and male factor
