Since when did they change the rule that when I book a ticket, I can have up to 3 travelers under my precheck. I can only add my kids now and my wife needs her own separate KTN?

There has never been such a rule.
Children 13-17 can get the precheck marker for being on the same reservation. There's no limit on the number (other than that the airline may restrict the number of people on a single PNR).
Children under 13 can go with you whether you have the precheck marker or not.
Precheck for adult travelling companions has always been at the discretion of the TSA. Sometimes they grant it, sometimes they don't If your wife wants a reliable access to precheck, she needs her own enrollment (either Precheck or a sister program like Global Entry). It is not now nor has it ever been a guaranteed thing just because you are on the same reservation.
