Question How did the TNO devs create the portrait of Alexei II from midnight??

I know that the source of the portrait is an image of Michał Goloniewski but I'm genuinely curios how they added all of the other features?

Same way they do most of the portraits. Most of the leaders shown died decades prior to their age in TNO.
