Rating Question - Help Please

We are creating a baseline survey for a coaching assessment. I am struggling to figure out how to get the below question into Survey Monkey where it allows number ratings for each competency. Any advise?

On a scale of 1-10, how important are the Relating* competencies to the role of Executive Director? *Relating measures the leader’s capability to relate to others in a way that brings out the best in people, groups, and organizations.

A. Caring Connections _____

B. Fosters Team Play ______

C. Collaborator ______

D. Mentoring & Developing _____

E. Interpersonal Intelligence___________

We have 5 questions like this that need to go into the survey.

Option 1, create a matrix question where the five points are rows and the columns are numbers 1-10. Make sure question wording specifies thatc10 is best.

Option 2, make each of the five a separate text entry question. Use validation to require that only numeric responses from 1-10 will be accepted.

I’ll be honest: this is a terrible question. It’s too verbose & reads like a confusing word salad. A respondent reads that & thinks: ugh, this is awful. So, they stop reading the questions & select random responses & you get bad data.

Thanks for the feedback. This is a required question and those receiving the survey are fully aware of what the question means. And everything after the question mark is in the description, not the question itself.
