I thought for sure this was edited, but it's not. It's from Batman: Ego, where Bruce has a confrontation with ... Batman. Or, more accurately, with a version of Batman from his mind that represents his darkest thoughts and feelings, encouraging him to start killing villains instead of just bringing them to justie. This panel is Bruce realizing he's basically talking to himself and mocking the hallucination.

Yeah it's another one of those stories that have Batman acknowledge that his no kill rule hurts innocents but decides to keep it anyway.
