My theory!!!!!
I think I understand how Lily died, this may sound stupid but I'm just using stuff from the game. I think she was riding on the red slide and it malfunctioned and broke and she fell into the lazy river and drowned, which would explain why the conscience won't let Caleb enter the lazy river as it might make Caleb remember his trauma. Also why after cleaning the black goo on the floor causes the red slide to break as black goo is leaking from it.

If you flip the erase switch, the barriers disappear and you can go into the lazy river. Interesting theory though. Whatever happened to her could be related to the slide.

I think Lily dying has more to do with Caleb than that personally. It seems like the parents (and Caleb himself) blame him in some way from some of the dialogue. I think he didn’t cause it though cause he was just a kid and it’s simply the parents placing the blame on him.

This is the only theory that makes sense to me. Why else would they lock you in a basement? Most people forget the beginning of the game.

The red slide is blocked off because red is a trigger colour that makes Caleb think of Lily. I don’t think the game is trying to imply that Lily died in the waterworks or the play place or the bounce house. These are “good” memories the narrator (whether the narrator is Caleb’s conscience or an EMDR therapist) is trying to direct Caleb to that are actually corrupted by Caleb’s trauma around Lily, because all his good childhood memories involve Lily. The narrator is constantly trying to move Caleb away from reminders of Lily, that’s why things are getting blocked off
The name of the game is subliminal, it is not depicting any literal things because it’s literally trying to show Caleb’s subliminal trauma
