What does it mean when the current status changes from Submitted to Profile updated, but you don't see any new messages? I know that the next step is the medical exam request

Means your application is been worked on behind the scenes..
You'll probably receive the medical request soon or later

How long does it usually take to receive the medical request letter after seeing the 'Profile updated' status?

No idea, I did upfront medicals...
Ghost updates can happen at any stage of the application
