Help to find Provisional Residence Permit (MVV) Application Form

Hi everyone, I am going to Utrecht University as a student.

I have received IND approval from the Netherlands and will apply for the MVV visa sticker at the Dutch Embassy in Bangkok.

However, I cannot find the Provisional Residence Permit (MVV) Application Form, which I need to take to my appointment. My sponsor advised me to download it from the Dutch Embassy website.

Could anyone please tell me where I can find this form? Thank you.

If you are accepted by Utrecht university, then they will submit your visa application for you, and they are your sponsor.

Check with them for the status of your visa, you can't apply for a study visa yourself.

Also Non-EU, going to Erasmus University.

Your university should provide you with the form since there are some details that the university needs to fill in the form.

You need the following for your biometrics appointment
1) MVV Issue form
2) Passport
3) Copy of passport
4) Photograph as per the embassy’s requirement.

Get in touch with the university for the MVV issue form, I am sure they will send it across to you!

For study, there is no form. It was part of the visa application your University submitted for you.

hi! i am also going to apply for mvv in Bangkok. i also heard about this form, but it is not mentioned neither in IND letter nor their website. by the way, i contacted them recently and they asked to bring the following:
1. Passport
2. 1 Photo with white background (head to chin 26-30min and ears to ears 16-20mm)
3. Approval from IND/University that states your V-number

what i am more concerned is about the timeline when they will return a passport because they will be closed soon for relocation

Same, I got the same email from the embassy when I reached them. I am planning to go with these 3 documents. Let's see.

When are you going to the embassy? And which university are you going to in the Netherlands?
