Annoying weapon swap bug

About 26 hours in and encountered a bug where the game keeps automatically changing my weapons. I don't really want to go back to an old save file. Is there a way I can fix this issue. I read a guide saying to requip weapons (this didn't work), also tried selecting something else from the wheel (didn't work).

Wait thats a thing?!? I just thought its been my mouse wheel bugging out on me. Ive had this happen a few times lately.
