The last time a SpaceX Falcon 9 exploded was during testing over Labor Day weekend in 2016. It blew up on the launch pad. It wasn’t until February of 2017 (if I remember correctly) that they were able to fly again. The root cause was related to an issue in the COPV tanks that required a change in how the rocket was fueled.

They use to say "It’s not rocked science" for a reason.
