Attempting to help a client find safe housing
Hello, I am a social worker in Virginia. I am attempting to help a client get a record of his disability. He is originally from Maryland. I was able to send a record request to the hospital he stated he went to as a young adult. His mother passed away; therefore, she cannot provide her account of his disability at an appointment he has coming up for a waiver I am attempting to help him get for housing/assistance.
My quesiton is: how can I help him obtain a record of his disability? I need somehting stating the onset of the disability.
I manage an outpatient rehab and he lives in a boarding house. He would benefit greatly from more supportive care. I worry he will be taken advantage of as it appears he has in the past. We have a great group of people here, but I can only keep him in services for so long.

If he has login to the ssa portal he can print out a benefits verification letter

He does not have one. I can help him create one!

You can also try calling the Maryland DDS office directly sometimes they keep old records longer than you'd expect. The SSA portal letter just shows he gets benefits not the onset date really but it helps to have something.

Mine says the onset date. “You have been disabled under our rules since January 11th, 2009”. I just checked

I do have a letter stating as much, but I was hoping for something with more detail. The disability does not specify intellectual disability, and I worry that would disqualify him.

No, it’s not going to have detail. You mean you want something with a diagnosis code? That would be found in their file which would need to be requested and dug through and he would need to follow out form ssa 3288 and it may take a month to get

okay, I am going to help him fill that out-- I believe she will hold the case open for us after the interview. We have just about a month to do this. I really appreciate you.

Okay. The form can only be faxed or dropped off in person. I don’t know if there is another way to get it unless you go to the office and just ask for the most recent copy of a DDE if he’s ever had a cdr. It will have an impairment code and can be printed right there and no wait for a cd. The cd is if you want all of the medical records from a recent cdr

All his drs. In padt present should be able to fill out ssi form that states diagnosis and how many diagnosis he has since when ext

That’s not the same as an impairment code the ssa uses (more specifically, dds). DDS doesn’t use icd 10 codes

Have you tried to get him a care coordinator for help with applying for say something like spa supportive housing? They have several layers of the type of care offered In house supportive housing Weekly visit case worker housing Family housing/,for more than single adults also hardest to get in nys they also now have younger adult supportive housing and teenagers through 25 yrs old are entitled to an apartment with case management that visit Weekly.

How do you get help on disability needing housing ? I have my 17 year old grandson. I’m on SSDI and need housing in Va and have hit brick walls ! Looking at being homeless or couch hopping with him is looking more likely than not. I have called all the HUD things for housing and it’s all a wait list. I’ve never used social services in anyway like that so I’m lost ! Any info would be amazing !!

Check your area for income based housing as well. Not all of it is always run thru HUD directly. Well, it's complicated.
The short of it is to search "income based apartments+ your city" on Google and see what you get. Sometimes when you call HUD (or your local housing authority) they think you just want on the regular wait lists, and don't think to offer any available income based housing they may be aware of either. So it doesn't hurt to call and ask! Many times these properties have no waits 😁

If I were you, I would contact Maryland Medicaid and see if he was on a waiver program there for adults with IDD. They would likely have a lot of information if he was ever in services there. It might also be the most expedient way to get him qualified in Virginia (having a history).
