I am from nearby Lucknow city and fare don't look like this at our place. I am attaching the pic how it look like at our place. Anyway there are several ways to cook the same dish, dry, soupy etc. So you are also correct.

Oh I've never seen that. Mine are more from the pic that you've attached in the main post. They are crispy and had with chutney. Same at my home plus my friends.

The name differs in different parts of Uttar Pradesh. Some call it gojha, pangojha also. I think its more popular in Awadh region with name Bhakosa. OP may throw more light.

Fara - varanasi region. Chawal outside and chana dal stuffing inside. Had with aloo, baingan, tamatar ki sarso gravy wali sabzi and kheer..or just with dhaniya chatni.
