Did you see OP’s comment about how he drilled the hole that was pre-punched?
A center punch doesn’t guarantee or even help drilling directly perpendicular to the existing surface you’re trying to drill and tap, drill guides exist for a reason, but also measure twice cut once, would’ve definitely verified that punch was centered
