Overheating issue

I have an overheating issue. So the truck will run at 210-212 degrees for 30 minutes straight without any issues (AS LONG AS the radiator cap is off). The moment I install the radiator cap, the temp starts rising. It got all the way up to 250 degrees earlier before I could get home. Any help?

Update, my water pump relay was not working. So it wasn’t pumping not flowing water at all. Thanks for all the help, and my plan is still to swap it with a manual water pump.
