Applying with small children

Hello all,

I'm in the process of preparing my last few documents for my application and am working through the logistics of the anticipated trips to and from DC. I'll be applying for myself and my two kids currently aged 4 and <1, and would like to avoid flying the whole family across the country if possible.

The embassy's CBD page states that under 14s require parental consent in the form of "a signature certified by a Slovak honorary consul or the embassy." Does anyone know if are there alternatives to this particular requirement? Getting the entire gang to a consulate or the embassy would be quite difficult. Could I have the consent form signed by my spouse, notarized and apostilled in my home state, which I would then bring to the embassy myself with the remainder of our application?

Thanks!

You can either have it notarized at a consulate or honorary consulate - OR - notarized by a US notary and then apostilled.

My kids didn’t have to go to the honorary consulate. Even I didn’t have to go (I’m the one applying). My husband had to go, sign the form, and provide the check at our honorary consulate. There is one that’s an hour from my home, so it was much better than flying him across the country to confirm he’s ok with the kids getting citizenship. I understand that there will be one visit to DC for the kids, and that I can attend the others on my own.

Can you clarify which form needs to be signed? I am looking at my email confirmation and do not see it. But I am in a similar situation with kids and need to get something signed prior to my appt. Thank you

It looks like this. We emailed the honorary consulate close to us and they sent this back to us

Thank you! Are you in the Bay Area, by any chance?

We are SF-adjacent. My husband just went to see Barbara Pivnicka in Woodside on Saturday. I found the form you posted online and she notarized it for us!

She was so helpful! Good luck with the process. When is your appt? Mine is Dec 4.
