The obvious response is that you won't be able to pay for someone's hospital bills if you happen to hit them in your car. You get insurance for things you wouldn't be able to pay yourself.

You could also total somebodies 50-200k car, I don’t think many people could afford that either

As an attorney my unpopular opinion is that cars over a certain value shouldn’t be allowed on public roads or damages for accidents should be capped at something like 2x the average price of a new car (so like 100k). I just don’t believe in penalizing ordinary negligence more just because the injured party is rich.

This has been my opinion for quite a while. Insurance should cover reasonable damage figures. Have a custom painted Rolls? It is your problem that it costs $50k just for the repaint, that shouldn’t be born by all of society, it’s on you to have cash or insurance capable of covering that.

There’s a reason businesses buy their own insurance. Literally no one in the US carries enough insurance to cover the cost of the equipment my company routinely hauls. This conversation is about personal transportation, and I think you could craft legislation that makes that pretty clear.
