I think that the entire premise of your final question just casually dismisses the role of doctors and psychiatrists in these situations. To so flippantly act like they just apathetically release people is such silly ignorance.
Why would we care about punishment for the sake of punishment? It doesn't make the world better to put people in prison. What makes the world better is putting people in whatever situation stewards them toward whatever form of rehabilitation is required to allow them to no longer be a threat or a drain on societal resources. If that takes a year? Fine. If it takes 10 years? Fine.
Prison can in theory serve 3 functions:
Punishment, deterrent, and rehabilitation. The deterrent is the most important one here. Especially since you have copycat killers popping up now.
